To sign up, your name and phone number are enough. You do not need to fill in a health questionnaire: the doctor will discuss this with you in person at the talk, and you will decide whether to take part after all the conditions have been explained to you.
What happens next. The coordinator will call you back during working hours (you can choose a convenient time in the form), tell you which studies are currently enrolling and, if you agree, book you in for a talk with a doctor. You are not committing to anything.
